Community of Christ and the Cedar Valley-Nauvoo Mission Center require each person (participant and staff) attending an event to complete and sign a Discipline, Liability, Medical and Photo Release Form. If any information changes after the form is submitted, please fill out and sign another form and submit it upon check-in at the event. It would also be helpful for a photo copy of both sides of your insurance card to be included.

Cedar Valley-Nauvoo Mission Center. General Information on Youth Camps. What to Bring to Camp:. Bedding – pillow, sleeping bag or sheets and blanket. Campfire blanket – something appropriate for use on the ground. Toiletries – soap, shampoo, toothpaste, etc. Food, beverages, and candy are not allowed in the cabins/housing. Clothing – warm/cool weather clothing, rain gear, extra footwear. Warm jacket or sweater. Grubby clothes for getting dirty, painting, etc. Other items that might be good to have:.

A: At this point in time, there must be a separate registration form for each camp, and for each child. Please send one form and fee per child for each camp you have a child or children attending.
