About The COACH Program. Why should I take part? The COACH Program is another way you can take action to prevent or delay the onset of type 2 diabetes or prevent complications from type 2 diabetes. A health coach can call you at home, work or mobile and at a time that suits you. The focus is on your risk factor targets for you condition, what you want to achieve and what you can actively do to improve your health. How does The COACH Program work? You will be part of The COACH Program for about 6 months.

How to join The COACH Program. Joining The COACH Program is quick and easy. Call us on 1300 136 588. Our helpful staff will take a few details from you and book you in for your first coaching session. Then just sign and send back the consent form we post out to you in the reply paid envelope supplied. You might have also been referred to The COACH Program by your GP or another health professional.
