However, once again, the NHS is having to answer questions about totally unacceptable care. At the heart of the problem, in my view, is 'whistle-blowing'. Staff know who are good colleagues and bad. Staff know when manning levels are dangerously low. Staff know when things are wrong and will usually know what it would take to put them right.
