Learning from patient encounters to make me a better doctor. Early warning, late realisation. Early warning scores are in use throughout NHS hospitals to spot patients who are deteriorating. So where did they come from, and what are their limitations? At Barts in 1997. Patients were selected for review by the PART based on a scoring system, where more deranged physiology led to a higher score. A certain score would trigger the nurse to call the ward doctor or the PART directly. What are the limitations?

Learning from patient encounters to make me a better doctor. Cerebral venous thrombosis is a real headache. 90% of headaches in pregnancy are migraine or tension-type headaches. Unfortunately, pregnancy is also a time of risk for two serious causes cerebral sinus thrombosis and idiopathic intracranial hypertension (which is often eventually diagnosed as cerebral venous thrombosis). How should the fact the patient is in the third trimester affect our management and investigations for headache? The nature ...

Learning from patient encounters to make me a better doctor. 3kg baby replaced by 3kg of urine. 32 L of urine replaces 3.2kg of baby. Our super SHO (not me) does a thorough examination discovers that there is suprapubic tenderness combined with the urge to pass urine. The patient feels really uncomfortable. A urinary catheter is passed. It drains 3.2L. How did this happen? During pregnancy (especially the third trimester), the bladder tone decreases and bladder capacity enlarges. Before she gives bir...

Learning from patient encounters to make me a better doctor. Pregnant women vomit a lot. I don’t just mean morning sickness. Pregnancy can result in altered taste and smell, which means certain foods and smells make them feel violently sick. Although most cases of pregnancy induced vomiting get better after 20 weeks, there are some unlucky ladies who feel sick every single day until the birth of their you-are-lucky-you-look-cute-otherwise-I’d-bloody. Little bundle of joy. Catecholamines are also released...

Learning from patient encounters to make me a better doctor. My A&E “go away” story. Thank you for sharing this – I’m a junior doctor and it has made me think about what kind of message we give patients based on the manner in which we discharge them from a&e. I’m normally a fan of Resilient GP. But today’s blogpost on how to manage ‘inappropriate demand’ in acute services. Patients who attend A&E or OOH inappropriately need to be told to go away. 1,365 more words. March 28, 2015. A child with fever.

Clinical mnemonics | Action Potential

https://apotential.wordpress.com/clinical-mnemonics

Skip to primary content. Archives (Med school so far). November 23rd, 2012. I’m going to try to only list mnemonics that I find helpful enough to actually use. This has the upside of keeping the list short and to the point, but the downside of potentially making me look like an idiot, since some of them are incredibly basic. I’m pretty much okay with that, though. Please feel free to comment with any additions! Smoking, Alcohol, Drugs — Occupation, Nutrition, Exercise). Have you ever felt you should C.

Practice laparoscopic skills at home to improve your skill and dexterity. You will need…. Monthly Archives: November 2013. It just came to my attention that a large number of users have iPhone instead of an Android phone. While I recommend using an Android phone (as I have personal experience of them), I understand that people may have invested (far too much) money into iPhones. Thus, when I get hold of an iPhone, I will write an equivalent tutorial on how to connect one to a PC. IP Webcam start screen.

Practice laparoscopic skills at home to improve your skill and dexterity. You will need…. I shall not detail how to set up the required PC programmes and smartphone App, as they should be self-explanatory. However, I will give one word of advice: charge your smartphone to 100% because the combination of both using the camera and LED and transmitting data over wireless network will rapidly drain the battery! Connect both your smartphone and PC to your wireless network. IP Webcam start screen. And see this...

Thoughts about being a patient, version 0.9. My A&E “go away” story. I’m normally a fan of Resilient GP. But today’s blogpost on how to manage ‘inappropriate demand’ in acute services. Filled me with dismay. It makes many points that I nodded along with, but then slips further and further into patient-blaming, culminating in:. 8220;Patients who attend A&E or OOH inappropriately need to be told to go away.”. In short – she told me to ‘go away’. This entry was posted in Uncategorized. December 30, 2014.

My place for random. The Pathway to Referral. The Pathway to Referral. A plea, and cautionary tale – please don’t disregard the differential on a blood count. When I was first diagnosed with Hodgkin Lymphoma, I privately wrote about my path to diagnosis. Reflecting on my ramblings, it is evident that I was upset, angry, and anxious all of which are perfectly normal reactions to officially receiving bad news . In the UK, the first port of call for any medical complaint is the GP (general/family practition...

I haven’t yet found anyone to make videos with, and I lack the laparoscopic experience to do so myself. In the meantime, watch this space, as since starting training in the East Midlands, I have started to become involved with MROG. I am pleased to announce my first video recorded using my DIY laparoscopy setup!

drcrunch | Learning from patient encounters to make me a better doctor

Learning from patient encounters to make me a better doctor. The seven month itch. A 7 month old boy has suffered with eczema for at least three months. It was all over his body, including the trunk, all four limbs and face. There was no flexural surface preference. On examination, there were areas of dry, lichenified skin. There were no blisters, crusts or vesicles. During the flares, he would itch frequently so was given chlorphenamine for night time relief. Do you think we should see a specialist?
