Your Health Matters is a project for adults with learning disabilities which was launched in April 2012. We work together with the NHS to make information about health. We make the information easy to read for people with learning disabilities. We look at ways to keep healthy so we can tell other people. We are supported by staff from the Involvement Team in the Learning Disability Service at Leeds and York Partnership NHS Foundation Trust. The project is open Monday to Friday. Our contact number is.
