Clinically relevant examples of use of these power vectors in finding differences between refractions, averages of refraction, and measures of dispersion and central tendency will be presented. These orthogonal power vectors are directly related to Zernike polynomials that are scaled by pupil size. 遠近両用コンタクトレンズ ＢＦＣＬ の有利な点である立体視の低下がない点を生かし、不利な点であるコントラスト感度低下を補う方法として、片眼にＢＦＣＬを装用し、対眼に通常の単焦点コンタクトレンズ ＭＦＣＬ を装用するModifiedモノビジョン法がある。 モノビジョンには特有の適応診断 症例選択 の難しさがあり、視機能 特に両眼視機能 の適応検査には細心の注意が必要である。 当院では１９９９年より取り入れてきたモノビジョンの基礎的、臨床的成果から機能的適応条件として 白内障以外の器質的眼疾患がなく、角膜乱視１ ５Ｄ未満であること 眼位検査にて斜視、斜位 １０⊿以上 がないこと 両眼視機能検査にて近見立体視１００秒以下であること 眼優位性定量検査にて眼優位性が弱いことが挙げられる。 Laser in situ keratomileusis(ＬＡＳＩＫ をはじめとする屈折矯正手術によるモノビジョンは、コンタクトレンズによる術後の見え方のシミュレーションが術前に可能であり、手術による広義のモノビジョンの中では比較的導入しやすい方法といえる。 The second part of the talk will fo...

Defocusの情報を、局所の網膜が検出し、その情報を脈絡膜を経て強膜の伸展につなげる 情報のカスケードには、不明な点が多いが、アトロピンのようなMuscarinic antagonistが、このカスケードのどこかに作用して 近視化を抑制すると考えられている。 そこで、現在臨床応用可能な近視進行防止のStrategyは、眼鏡またはコンタクトレンズ CL による遠視側defocusの防止と、 Muscarinic antagonistの点眼が考えられる。 軸外収差 周辺部網膜のdefocus 予防の眼鏡、CLに関しては、近視化が30 程度抑制 されるという報告があり、個人に応じた矯正が可能になれば、有効な近視抑制法となる可能性がある。 一方アトロピンより散瞳作用が弱い、Muscarinic antagonistのピレンゼピン点眼薬を使用して、健常若年成人に対して副作用の チェックを行ったところ、濃度１ までは瞳孔径、調節力に有意な変化はないが、近視抑制に有効とされる２ では、散瞳、調節麻痺が 有意に認められた。

その後、疼痛がなく視力回復が早いLaser in situ keratomileusis(LASIK)の登場により、屈折矯正手術の中心はPRKからフラップ下ablationのLASIKへと移行した。 Surface ablationは、アルコールを使用して角膜上皮のフラップを作るLaser epithelial keratomileusis(LASEK)が考案されPRKの欠点が改善されたが、手技の煩雑さやアルコールによる影響で角膜上皮細胞の一部死滅が問題であり、機械的にエピケラトームで角膜上皮のフラップを作るEpipolis laser in situ keratomileusis(Epi-LASIK)へと移行した。 当院でのPhakic IOL経験からArtisan IOLとArtiflex IOL移植術を、角膜矯正手術のLASIKおよびPRK手術の経験と比較した。
