What Is Required to Receive WIC. To be eligible to participate in the WIC Program, one must:. Reside in Nevada or on Nevada Tribal Land. Be a pregnant or recently pregnant woman, infant or child up to age 5. Have a moderately low income (see chart below). Be determined to have a nutritional risk (see sidebar). How to Apply to the WIC Program. If you or someone you know or work with might benefit from WIC services, please call (775) 398-4960. At each certification appointment requires the following items to bring with you:. Proof of residency (i.e. Power/NV Energy Bill, Rent/Lease, Telephone Bill or a Bill with a physical address, NO P.O. Boxes). Proof of Income for everyone in the household (i.e. Paystub Within 30 days, SNAP Program, TANF, and Medicaid Letter we will not accept the Medicaid cards).
