Project Independence, an aging in place initiative, helps the Town of North Hempstead's older residents connect to health and mental health services. The program links seniors to providers of physician home visits, geriatric mental health assessments, Alzheimer's Disease support programs and advance directive education. Our partners provide case management, supportive counseling, wellness classes and nutrition education. For more information, or to register for Project Independence, call 311 or 869-6311.

Caregivers are special people. A caregiver may be a spouse, an adult child, a sibling, a close relative or friend, or a paid service provider. Some caregivers provide hands on care, supervise the home situation, manage finances and/or provide transportation. They offer the emotional/physical support vital to keeping a loved one at home.
